The U.S. Chemical Safety and Hazard Investigation Board (USCSB), generally referred to[2] as the Chemical Safety Board (CSB), is an independent U.S. federal agency charged with investigating industrial chemical accidents. Headquartered in Washington, D.C., the agency's board members are appointed by the president and confirmed by the United States Senate. The CSB conducts root cause investigations of chemical accidents at fixed industrial facilities.[3]
